Psychiatric Consultant

The Psychiatric Consultant (PC) will provide consultation to the Behavioral Health Home (BHH) team, offering guidance on evidence based practices and strategies to incorporate into the Plan of Care. BHH is a community based service, offering comprehensive case management and support services to individuals that emphasize self-management of physical and behavioral health conditions.

This is a contracted position on an as-needed basis consisting of up to 4 hours per month, including attending one BHH Team meeting per month (in person or by Zoom). The majority of consultation would be by phone or email with the Clinical Supervisor and/or the Program Manager for the BHH Program who would reach out to the PC as needed. Position is located in Bangor/remote.

Minimum Qualifications: licensed psychiatrist or eligible for examination by the American Board of Psychiatry; or has completed three years of post-graduate training in psychiatry; or a PMH-APRN.

Family/Youth Peer Support Specialist

The Certified FYSS provides community-based support to families of children who have a mental health diagnosis and need some support/assistance with managing challenges and navigating care systems. The FYSS must be an individual who has, or has a family member who has, received, or is currently receiving, supportive services related to a diagnosed mental illness and is willing to self- identify as such. Opening in Bangor.

This position requires CIPSS certification (or eligibility) and a minimum of a high school diploma. You must also have a valid State of Maine driver’s license.

Early Childhood Mental Health Consultant (ECC)

Visit Support Worker

Community Care is excited to announce that we are now providing supervised visitation services for families! If you have been looking for a chance to make a direct and lasting impact on the lives of children and their families, this is the position for you!
A work mobile phone is provided for VSW’s to better assist you in this position.

We are seeking several part-time Visit Support Workers (with the potential for full time depending on your availability/flexibility) to supervise visits between youth who are in the custody of the Department of Health and Human Services and their family members. The VSW supervises visits, providing structure and guidance to visit participants, in one of our Family Visitation Centers, within families’ homes, or within the community as determined by the Family Plan. The VSW plays an important role in helping families work toward reunification. VSW’s must have reliable transportation and a State of Maine driver’s license to provide transportation for youth to and from visits as necessary. Multiple shifts are available with peak hours being between 2pm and 7pm Mon-Fri. This is an ideal position if you are looking to supplement your current income and/or to have shifts that can accommodate your school or work schedule. We are also flexible with the number of hours worked per week!

Adult Services Case Manager

We are seeking caring and motivated individuals to provide community integration for adults coping with mental health symptoms in the Bangor area. Contracted Clinical Therapist

The clinician will provide a high level of clinical support to youth and families through our Mental Health Clinic. The clinician will also have the opportunity to provide outpatient mental health services to a diverse group of individuals including adults, adolescents, and children by providing assessments and ongoing mental health services. Must be able to meet the diverse clinical needs of a broad range of individuals of various ages. This is an ideal position if you are looking for 10 or more hours per week in an office environment where you can set your own schedule and determine your own caseload but with administrative support available on-site to assist you. The rate for this position is $46/hour for direct care services.  
Minimum Qualifications: LCSW, LMFT, LCPC, or Ph.D., Currently licensed or otherwise recognized as qualified by the state of Maine in the disciplines required for the children served by the program.  Will accept LMSW Conditional II or LCPC Conditional under the provision that individual will be fully licensed within the time frame defined by supervisor. M.S./M.S.N. (masters level psychiatric nurse) licensed in the State of Maine (or eligible).  Experience supporting children and families in either a paid or volunteer setting.

Family Visitation Program Supervisor

Community Care is seeking a full-time Supervisor to supervise the Family Visitation Program in York County, Cumberland County, and another in Rockland. The Supervisor will be responsible for supervising and supporting Visit Support Workers, reviewing and assigning visitation referrals, assisting in supervising visits when necessary, overseeing the visitation centers, reviewing documentation, and working as a member of a team to manage the Family Visitation Program. They will have availability during the peak hours of 2-7pm Monday through Friday, with the flexibility to adjust work hours as necessary to meet the needs of the program. Outreach Worker-Shaw House

Community Care is seeking a full time or part time candidate for their Street Outreach Program located through Shaw House, a program designed to locate and serve homeless youth within the community. The candidate will work flexible hours and will have the ability to make adjustments to his/her schedule as necessary in order to accommodate the needs of the program. The Outreach Worker will conduct activities designed to reach youth who are experiencing homelessness including, but not limited to, walking on various terrain throughout areas of the communities we serve to locate areas where homeless youth may be congregating, providing support services to youth, and assisting to connect youth with services. Since Outreach occurs year round, the ability to work outside during all seasons is required. Travel is required, using the agency vehicle. The Outreach Worker will establish relationships with youth to ensure they are connected to longer term community resources and will encourage youth to participate in pro-social activities in their communities. The Outreach Worker will also attend community meetings and build partnerships with other agencies/service providers to assist in meeting the youths’ needs.

This position requires 1 year of experience working with at-risk youth and a minimum of a high school diploma. You must also have a valid State of Maine driver’s license.

Youth Transition Coordinator

The Youth Transition Coordinator (YTC) partners with youth experiencing homelessness as they work to achieve safety, stability and permanency. YTCs are skillful in quickly establishing rapport with youth: collaborating with them to develop a plan of care consisting of clear, measurable goals that not only address their housing needs but also serve to strengthening their family relationships, address mental and/or physical health needs, educational needs, and employment opportunities. The YTC also plays an active role in engaging houseless and runaway youth in conversations geared towards harm reduction, healthy relationships, overall support, and advocacy.  YTCs participate in a variety of diversion activities, including family mediation and advocacy work within their assigned communities.

Valid State of Maine driver’s license, Bachelor’s degree and at least 1 year of case management experience is required. *For the Dover-Foxcroft position, relevant experience could be substituted for a Bachelor’s degree.

Regional Outreach Coordinator

Community Care is seeking a Regional Outreach Specialist (ROS) to work within the Street Outreach Program. Street outreach services are provided directly to youth, and through collaboration with other agencies. The ROS will work closely with other organizations to protect and treat young people who have been, or are at risk of, sexual abuse or exploitation. Street outreach services include:

· Street-based education and outreach
· Access to emergency shelter
· Survival aid
· Individual assessments
· Trauma-informed treatment and counseling
· Prevention, education and harm reduction activities (alcohol and drug abuse; sexual exploitation; sexually transmitted infections, including HIV; and physical and sexual assault)
· Information and referrals
· Crisis intervention
· Follow-up support

Community Care utilizes a positive youth development (PYD) framework and a trauma-informed care (TIC) approach. PYD suggest that the best way to prevent risk and trauma is to help young people achieve their full potential. Youth development strategies focus on giving young people the chance to exercise leadership, build skills, and become involved in their communities.

ROS’s are most successful when they are approachable, self-aware, accepting, fun, adaptable, engaging and committed to supporting self-determination for all, unconditionally.

Clinical Manager for Treatment Foster Care

The Clinical Manager will serve as the direct supervisor for Community Care’s Treatment Foster Care (TFC) Program staff including direct support providers and case managers. In addition, the Clinical Manager for TFC includes a variety of responsibilities to ensure the clinical integrity of the program. Children that are in DHHS custody and struggle with significant emotional and behavioral needs as a result of trauma often require a higher level of clinical care. The Clinical Manager is available to provide clinical support and information to treatment level foster parents, children in the program, as well as TFC staff. This includes clinical assessment of children receiving the service, providing guidance and support around what therapeutic interventions are recommended, helping guide appropriate referrals, completing Vineland Assessments, and addressing educational issues and needs. Clinical training and support is also provided to TFC foster parents regarding specific diagnostic issues related to the children in their homes as well as larger topics including the Trauma Informed Parenting curriculum. The Clinical Manager will also ensure that staff are in compliance with all paperwork and that monthly contact obligations are met as well as facilitate clinical group supervision for staff. Clinical Manager will also stay up to date on Statewide TFC initiatives and potential training needs of the program.
Minimum Qualifications: LCSW, LMSW-cc, LCPC, LCPC-c, LMFT

Outreach Manager

The Outreach Manager (OM) provides hands-on oversight of the operations of the Outreach Program both locally and regionally; assures compliance with all programmatic and regulatory standards and requirements; exercises effective program strategies and maintaining sound Outreach practices. Community Care has multiple funding sources that support Outreach activities and each one has specific parameters which must be understood and met.

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed are representative of the knowledge, skill, and/or ability required.

This position is located in Bangor but travel throughout the coverage area is required.

ESSENTIAL JOB FUNCTIONS:
1. Provide direct supervision of program staff to ensure the provision of quality, cost effective services.
2. Ensure provision of client services related to Outreach and community Diversion and Navigation.
3. Develop and maintain positive relationships with relevant community authorities and resources.
4. Coordinate file reviews on all client files, at least monthly to ensure regulatory compliance.
5. Attend external community meetings as assigned with the Executive and/or Asst. Director and attend/participate in other internal staff/team meetings as assigned.
6. Oversee, seek out, and actively engage in public relations campaigns and Community Education for the Outreach Program.
7. Ensure contractual compliance and grant management of all funding sources as assigned.
8. Provide a direct link and support communicating other Community Care services.
9. Develop and oversee a plan of assistance with each client, which incorporates a youth development approach to their problem solving.
10. Present in a manner that is approachable, self-aware, accepting, fun, adaptable, engaging and committed to supporting self-determination for all, unconditionally.

MINIMUM QUALIFICATIONS
A Bachelor’s degree in Human Services or related field plus 2 years of experience in case management or an Associate’s degree in a related field plus 4 years of experience in case management or a related field working with youth. Additionally, a minimum of one year of experience as a supervisor or performing supervisory activities.
