Family/Youth Peer Support Specialist

The Certified FYSS provides community-based support to families of children who have a mental health diagnosis and need some support/assistance with managing challenges and navigating care systems. The FYSS must be an individual who has, or has a family member who has, received, or is currently receiving, supportive services related to a diagnosed mental illness and is willing to self- identify as such. Opening in Bangor.

Early Childhood Mental Health Consultant (ECC)

Visit Support Worker

Community Care is excited to announce that we are now providing supervised visitation services for families! If you have been looking for a chance to make a direct and lasting impact on the lives of children and their families, this is the position for you!
A work mobile phone is provided for VSW’s to better assist you in this position.

We are seeking several part-time Visit Support Workers (with the potential for full time depending on your availability/flexibility) to supervise visits between youth who are in the custody of the Department of Health and Human Services and their family members. The VSW supervises visits, providing structure and guidance to visit participants, in one of our Family Visitation Centers, within families’ homes, or within the community as determined by the Family Plan. The VSW plays an important role in helping families work toward reunification. VSW’s must have reliable transportation and a State of Maine driver’s license to provide transportation for youth to and from visits as necessary. Multiple shifts are available with peak hours being between 2pm and 7pm Mon-Fri. This is an ideal position if you are looking to supplement your current income and/or to have shifts that can accommodate your school or work schedule. We are also flexible with the number of hours worked per week!

Adult Services Care Coordinator

We are seeking caring and motivated individuals to provide case management services for adults coping with mental health symptoms in the Bangor area. This position provides case management consultation, skill teaching, development of strategies to make family life less stressful, and advocacy/coordination for service needs in an effort to reduce cycles of instability in the lives of those affected by mental illness.

Contracted Clinical Therapist

The clinician will provide a high level of clinical support to youth and families through our Mental Health Clinic. The clinician will also have the opportunity to provide outpatient mental health services to a diverse group of individuals including adults, adolescents, and children by providing assessments and ongoing mental health services. Must be able to meet the diverse clinical needs of a broad range of individuals of various ages. This is an ideal position if you are looking for 10 or more hours per week in an office environment where you can set your own schedule and determine your own caseload but with administrative support available on-site to assist you. The rate for this position is $46/hour for direct care services.  
Minimum Qualifications: LCSW, LMFT, LCPC, or Ph.D., Currently licensed or otherwise recognized as qualified by the state of Maine in the disciplines required for the children served by the program.  Will accept LMSW Conditional or LCPC Conditional under the provision that individual will be fully licensed within the time frame defined by supervisor. M.S./M.S.N. (masters level psychiatric nurse) licensed in the State of Maine (or eligible).  Experience supporting children and families in either a paid or volunteer setting.

Family Visitation Program Supervisor

Community Care is seeking a full-time Supervisor to supervise the Family Visitation Program in York County and another in Cumberland County. The Supervisor will be responsible for supervising and supporting Visit Support Workers, reviewing and assigning visitation referrals, assisting in supervising visits when necessary, overseeing the visitation centers, reviewing documentation, and working as a member of a team to manage the Family Visitation Program. They will have availability during the peak hours of 2-7pm Monday through Friday, with the flexibility to adjust work hours as necessary to meet the needs of the program. The Youth Transition Coordinator (YTC) partners with youth experiencing homelessness as they work to achieve safety, stability and permanency. YTCs are skillful in quickly establishing rapport with youth: collaborating with them to develop a plan of care consisting of clear, measurable goals that not only address their housing needs but also serve to strengthening their family relationships, address mental and/or physical health needs, educational needs, and employment opportunities. The YTC also plays an active role in engaging homeless and runaway youth in conversations geared towards harm reduction, healthy relationships, overall support, and advocacy.  YTCs participate in a variety of diversion activities, including family mediation and advocacy work within their assigned communities.

The Youth Permanency Specialist (YPS) partners with youth enrolled in housing-based services as they work to maintain safety, stability, and permanency. Family Time Coach

The Family Time Coaching Program serves families who are referred by DHHS/OCFS, whose children have recently entered the custody of DHHS. The FTC provides supervised visitation in family’s homes/communities utilizing a coaching model to promote healthy relationships and in accordance with each individual family’s Family Visitation Plan. The FTC serves as a team player on the family’s team, consistently demonstrating unconditional positive regard, compassion, and empathy.

ESSENTIAL JOB FUNCTIONS:

  • Provides supervised visitation for parents and children in the family’s home or community.
  • Provides feedback and coaching to parents to promote healthy familial engagement.
  • Thoroughly reviews each Family Visitation Plan, referral, and court orders to ensure visits occur within established guidelines.
  • Attends Family Team Meetings to partner with family members and to keep team members abreast of progress occurring within visits.
  • Completes visitation notes in accordance to program procedures.
  • Coordinates and engages parents in pre/post discussions to take advantage of windows of opportunity for teaching and de-briefing in order to assist parents in strengthening parenting skills.
  • Ensures culturally competent service delivery, evaluating the cultural preferences and needs of each family and customizing visits accordingly, as appropriate with the Family Visitation Plan.
  • Responds to subpoenaed requests, attending court as necessary.
  • Offers flexibility to meet the needs of the FTC Program, including ensuring availability when families are able to meet.
  • Consistently role models a high level of professionalism during interactions with co-workers, supervisors, and external providers.
  • Travels throughout the District as necessary to meet the needs of the FTC Program.

Supervisor-Family Time Coaching Program

This position will cover Cumberland and York Counties

ESSENTIAL JOB FUNCTIONS:

1. Provides consistent and quality supervision to Family Time Coaches (FTC’s).
2. Thoroughly reviews each Family Visitation Plan, referral, and court orders to ensure visits are provided within established guidelines.
3. Provides initial and ongoing training to FTC’s.
4. Provides support to FTC’s who are delivering intensive, home and community based skill building to parents of children in DHHS custody.
5. Assists FTC’s and families in the development of visitation goals and activities.
6. Provides coverage for FTC’s as necessary, including providing in home visitation.
7. Follows through with all programmatic training requirements.
8. Ensures notes and reports are submitted in a timely and accurate manner by FTC’s.
9. Attends FTM’s as necessary to partner with family members, FTC’s, and other providers.
10. Attends collaborative meetings with DHHS.
11. Contacts the Program Director immediately with any safety concerns.
12. Ensures the FTC’s are providing culturally competent services, evaluating the cultural preferences and needs of each family and customizing visits accordingly.
13. Responds to subpoenaed requests, attending court as necessary.
14. Meets all expectations of the FTC Supervisor job description.
15. Initiates professional development through reflecting on areas of strengths and challenges, and seeks assistance in supervision to strengthen skills.
16. Adheres to the agency’s clinical, administrative, and personnel polices and procedures.
17. Offers flexibility to meet the needs of the FTC Program, including ensuring availability when FTC’s need support.
18. Consistently role models a high level of professionalism during interactions with FTC’s, co-workers, supervisors, and external providers.
19. Travels throughout the assigned counties as necessary to meet the needs of the FTC Program.

Minimum Qualifications:

  • At minimum, shall be a Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), or Licensed Clinical Social Worker-conditional (LCSW-conditional)
  • At least 12 months of previously documented experience working with children and families.
  • Valid State of Maine driver’s license, 100/300 automobile insurance, and a reliable vehicle
